*Your personal data is required for insurance purposes and will be held securely by Greenpower at their head offices to be shared with Motorsport UK (insurers) and/or relevant third parties should the need arise in the event of an accident. All personal data (including gender) will be destroyed 3 years plus 6 months (or until the individual is 18 years of age, then another 3 years and 6 months) after the end of the Greenpower race season.

As the team leader you are acting in loco parentis and have responsibility of the participants on the day. When completing this form, please gather the necessary information so you can be confident in your answers, namely regarding the wellbeing and fitness of each child to participate.

There is no requirement for a physical signature, your agreement to these terms is enough.

Where the participants are aged under 18 at the date of event, the following risks must be explained to them and their parents/guardians. A person with Team Leader Authority can then complete this form in loco parentis for all participants. Please email info@greenpower.co.uk for more information.

As the team leader you will have the following details for every team member at the event: Emergency contact name, Emergency contact phone number, Home address. These details must be provided to Greenpower Staff or other relevant parties at any point during the event should it be required.

I confirm that I am satisfied (by sighting lap or otherwise before taking part) that the venue and track is acceptable to me with regards to its features and physical layout.

By taking part, my team and I are aware and accept the risks involved in the event including the risk of injury, disability, death and property damage.

My team and I are aware and accept that other participants will be on the track at the same time.

My team and I are free of alcohol and drugs (prescribed or otherwise) and understand not to consume any substances during or prior to any event that could impair ability to participate.

My team and I have read, understood, and will comply with all circuit instructions, regulations and final instructions for this event given by the organiser and understand any reckless/dangerous driving is strictly forbidden and will result in my immediate dismissal from the event.

My team and I have seen and understood the pre-race information that has been sent to me prior to this event. I understand that failure to comply with these procedures could result in disqualification. My team and I will also attend all driver briefings on the day of the event and comply with the information given.

Indemnity: In consideration of the acceptance of this entry I agree that neither any one of or any combination of Motorsport UK and its associated clubs, the organisers, the track owners or other occupiers, the promoters and their respective officers, servants, representatives and agents (the “Parties”) shall have any liability for loss or damage which may be sustained or incurred by me as a result of participation in the Event including but not limited to damage to property, economic loss, consequential loss or financial loss howsoever caused. Nothing in this clause is intended to or shall be deemed to exclude or limit liability for death or personal injury. To the fullest extent permitted by law I agree to indemnify and hold harmless each of the Parties in respect of any loss or damage whatsoever and howsoever arising from my participation in this Event.

My team and I have been given the opportunity to read the General Regulations and all Official Documentation and will abide by them. I declare that my team and I are physically and mentally fit to take part in the event and my team and I are competent to do so. I acknowledge that I my team and I understand the nature and type of competition and the potential risk inherent with motor sport and agree to accept that risk.
